Athletic Ride Home Authorization Form

This form is to be used to authorize individuals (other than parent or legal guardian's) to transport Cavelero athletes home from school activities held AWAY from the Cavelero campus. It is valid for a single sports season. Form is to be completed and returned to your athlete's coach directly.
